Standard Requirements]

(1) Preferred business name along with 3 alternative names

(2) Application form, in duplicate

(3) P 15.00 worth of documentary stamps

(4) DTI Pro-Forma Report on Capitalization and Employment Generated

(5) Two 2” x 2” size pictures

[Additional Requirements]

1. Proof of Filipino Citizenship, for owners who are:

  • natural-born Filipinos whose name is suggestive of a foreign nationality (e.g. birth certificate, voter’s ID)
  • naturalized Philippine citizens (e.g. photocopy of naturalization certificate, of allegiance, ID issued by the Bureau of Immigration and Deportation)
  • Filipinos by election (e.g. photocopy of affidavit of election, ID issued by the Bureau of Immigration and Deportation)

2. For corporations/partnerships which opt to register their business names:

  • Certified true copy of the Articles of Incorporation/Partnership, By-laws and SEC Registration Certificate
  • If equity is more than 40% foreign, the SEC certificate must specify that the registration is in accordance with the Foreign Investments Act of 1991

3. For cooperatives that opt to register their business names, a copy of the registration with the Cooperative Development Authority

4. Latest Franchise Agreement for franchise holders

5. For foreign investors:

  • Alien Certificate of Registration
  • Accomplished DTI Form 17 under R.A. 1180
  • Written appointment of Filipino resident agent
  • Proof of inward remittance of foreign currency, for non-resident alien, or Bank Certificate of Deposit, for resident alien
  • Authority to verify bank account/Bank Certificate of Deposit
  • Copy of Valuation Report from the Bangko Sentral ng Pilipinas (BSP), if investment includes assets other than foreign exchange
  • Certification from resident alien not seeking remittance of profits
  • Clearance from other government agencies, if required
  • For enterprises involved in the retail business, Permit to Engage in a Retail Business as per R.A. 1180
