a. Accomplish Waiver form for new connection. Wait for results of
inspection.

b. Excavation permit from CEO if groundwork will be done within
city government limits or DPWH for national hi-ways and roads.

c. Mayor’s permit/Business License

d. Lot owner’s authorization if location is within a subdivision

e. Line owner’s authorization for sub-connection
