ABOUT THE SERVICES

THE CITY HEALTH Office issues a Sanitary Permit to operate in all business establishments after the actual inspection. Health cards are being used to operators and employees after physical examination and after attending the Food Handlers Class to all food handlers.

1. Present Official receipt for health certificate fee to the person-in-charge 1 minute LEONIDA C. LEGASPI

2. Assess completeness of requirements. 1 minute SANITARY INSPECTOR
3. Register Client and purpose of issuance 2 minutes SANITARY INSPECTOR
4. Accomplish Health Card, Sanitary Permit Form and Business License. 4 minutes SANITARY INSPECTOR
5. Submit accomplish forms to City Health Officer for evaluation and signature. 2 minutes DR. VITO C. BORJA II
