Registration of Death Certificates

ABOUT THE SERVICE

THE SPOUSE or nearest relative who has knowledge of the death of a person-who died without medical assistance-must report the same to within 48 hours.

The City Health Office examines the cause of death, signs the death certificate, and directs the registration of the death certificate with the Office of the City Civil Registrar within the reglementary period of 30 days.

HOW TO AVAIL OF THE SERVICE

FOLLOW THESE STEPS IT WILL TAKE YOU PLEASE APPROACH
1. Presentation of Documents

For deaths occurring outside hospitals, clinics, etc.

(Approach the employee-in-charge at the City Health Office who will prepare the death certificate of the deceased. Present a Certification from the barangays where the deceased died.)

Submit the duly accomplished death certificate  to the office for registration

For deaths occurring in hospitals, clinics, etc.

Present the death certificate, certified by the attending physician, from the hospital, present it to the EIC.

2. Examination of document

Employee-in-charge examines the documents presented.

Employee-in-charge checks if it is properly filled-out.

3. Review by the City Health Officer

Registrant is course to the City Health Officer who will review/examine then sign the document. He will certify as to the cause of death.

4. Payment of Fees

Pay the corresponding fees at the City Treasurer’s Office

If the client is an indigent, he is advised to avail of the free burial assistance being given by the Mayor’s Office

5. Registration of Document

Registrant returns to the EIC who will assign a registry number to the document.

6. Signing of the Registered Death Certificate.

The Civil Registrar signs the registered death certificates.

7.  Release of Document
