Requesting Endorsement of Registry Records to Civil Register-General

ABOUT THE SERVICES

AS A RULE, all civil registrars shall submit civil registry documents to the Office of the Civil Registrar-General (OCRG) through their respective Philippine Statistics Authority (PSA) provincial offices.

There are instances when the PSA National Office cannot issue pertinent documents to interested parties because records are namely:

v  Not available in its archives

v  The requested documents are still being processed by the provincial office

v  With blurred or off-line entries

v  Correction of entries in the Geographic Statistical Portion and/or Registry Numbers on Civil Registry Documents

To facilitate the issuance of requested documents, the concerned Provincial Statistics Officer (PSO) or Civil Registrar is required to submit or indorse the needed document to the PSA.

  • PSA Negative Result Certification     or
  • PSA issued Birth/Marriage/Death Certificate

Endorsement Fee P300.00
Certified copy of Birth/Marriage/Death Certificate P150.00

HOW TO AVAIL OF THE SERVICES

FOLLOW THESE STEPS IT WILL TAKE YOU PLEASE APPROACH
1. Submit Request for verification

Approach the employee-in-charge (EIC) and request for an endorsement of your record to PSA.

Show your PSA Negative Result Certification/PSA issued Birth/Marriage/Death Certificate. This must have been issued within the past 6 months.

2.Verification

EIC verifies from the computer database whether the requested record is available.

If the record is not available in the database, a manual search will be done in the archives. If the search will proved futile then you will be advised to apply for late registration.

If the record is available, your document will be processed for Endorsement.

3. Payment of Fees

You will be instructed to pay the required fees

4. Preparation of Endorsement

EIC prepares a true copy of the document, as well as an Endorsement letter to the PSA.

The Civil Registrar reviews then signs the documents.

6. Release of Documents

EIC releases the documents to client.
