THIS SERVICE is requested to annotate or cancel annotation on mortgage, levies etc.

  • Copy of the Instrument/Release documents

  • Research Fee – P 50.00 per document

HOW TO AVAIL OF THE SERVICE

FOLLOW THESE STEPS IT WILL TAKE YOU PLEASE APPROACH
1. Request for the Service

Inform Officer- of- the Day (OD) regarding the request. OD, in turn, provides a Service Slip for endorsement to the Records.

2.Submit Service Slip

–> Submit the Service Slip with required documents to the Records Chief and wait for the request to be evaluated.

–> Records Chief assigns task to Admin Aide

3. Annotation of Tax Declaration

–> Admin Aide annotates and prepares the Tax Declaration (TD) for signature of the City Assessor.

–> The City Assessor checks and signs the TD

4. Issuance of Annotated TD

–> Receive a copy of the annotated TD.
